What Is Yellow Fever?

Yellow fever is a mosquito-borne, viral hemorrhagic illness. Yellow fever can show itself in a variety of ways ranging from asymptomatic illness to acute-onset viral hepatitis and hemorrhagic fever.


How Do I Get Infected?

Monkeys serve as animal reservoirs of the virus and it is transmitted to humans via various species of mosquito.  The mosquito is infected when it sucks the blood of an infected monkey and it develops a permanent infection that  accumulates in its salivary glands. The virus is introduced into the human bloodstream via the saliva of the mosquito when it bites the human.  Urban-type epidemics of yellow fever can arise when unvaccinated humans come into contact with domestic mosquitoes. Yellow fever virus is not transmitted person to person.

 

Where Does Yellow Fever Occur?

The vast majority of cases and deaths take place in sub-Saharan Africa, where yellow fever is a major public health issue. Thirty-two African countries (including amongst other Kenya, Nigeria and the Congo) are now considered as areas with risk of yellow fever transmission as well as ten South and Central American countries (Including amongst others Brazil, Colombia and the Pananma) and the Caribbean.  Bolivia, Brazil, Colombia, Ecuador, Peru and Venezuela are considered to be at greatest risk.  Yellow fever has never been reported from Asia.  Certain countries will not allow you access without presentation of a Yellow Fever Vaccination Certificate.

Symptoms and Signs of Yellow Fever

The incubation period is short, usually 3 - 6 days.  Typical cases are characterised by:
  • Sudden onset fever, backache, generalised muscle pain, prostration, nausea and vomiting.,
  • Slow, weak pulse may be felt, that is out of proportion to elevated temperature.
  • Jaundice is moderate in early disease but intensifies later.
Most infections resolve at approximately 5 days.   Some cases may progress following a short remission of hours or a day to severe disease. Haemorrhagic symptoms including gingival bleeding, haematemesis, melaena, and liver and renal failure. 20%-50% of jaundice cases are fatal. Recovery from yellow fever is followed by lasting immunity; second attacks are unknown

 

How is Yellow Fever Diagnosed?

A correct diagnosis of yellow fever requires a high degree of suspicion based on a history of travel to an endemic area, as well as a vaccination history. There are specific serological blood tests to detect Yellow Fever.

   

Treatment and Prophylaxis

There is no specific anti-viral therapy and treatment is symptomatic. Mortality is high, up to 60%.

 

Which Vaccine does MediClinic offer?

MediClinic is a NATHNAC designated Yellow Fever Vaccination Centre and offers STAMARIL (Sanofi Pasteur MSD).  A single dose offers 10 years protection. Protective antibodies form within 7-10 days.  A booster dose is required every 10 years.

 

Patients who are allergic to eggs or chicken proteins and who are immunocompromised should NOT be vaccinated with this vaccine.

 

For elderly travellers, the risk for severe illness and death due to Yellow Fever infection should be balanced against the risk for systemic illness due to Yellow Fever vaccine. The possible side effects and serious reactions will be discussed at the time of consultation.

 

MediClinic will provide an International Certificate of Vaccination or Prophylaxis (ICVP) following vaccination .

Things to remember: 

  • You should be well when you have your travel vaccines. If you are unsure or suffer with any symptoms suggesting illness, then please call us before your consultation.

  • Travel Vaccination documents – if you have had previous travel vaccinations please bring the relevant documentation to your appointment, this will help us ensure you are fully protected during your travels.
  • We recommend vaccines be given 1 week to 3 months prior to traveling depending on the vaccine. This will allow your body sufficient time to build the necessary antibodies to protect you.
  • Cancellation of appointments must be made at least 24 hours prior to the appointment to avoid a cancellation fee.
